General acknowledgement

I acknowledge that I must adhere to The Neon Salon’s policies. I understand that cancellations must be done with at least 24 hours’ notice. Failure to do so will result in the loss of a package or 50% of the total service cost. I acknowledge that ANY no show will result in the loss of a package or 100% of the total service cost. I understand that after 15 minutes of tardiness my appointment may be subject to cancellation, or my treatment time will be cut short, and I will be responsible in accordance with the “No-show” policy.

I acknowledge that my skin might experience temporary irritation, tightness, redness, or slight swelling which usually dissipates within 72 hours depending on skin sensitivity. 

I acknowledge that if I am allergic to one or more ingredients in the products used, I may experience allergic reactions.

I acknowledge that if I fail to use a minimal sunscreen (SPF45), I am more susceptible to sunburn, skin damage & hyperpigmentation. I should avoid excessive sun exposure especially between 10am-2pm.

I acknowledge that this treatment is a strictly elective cosmetic procedure and no medical claims have necessarily been expressed or implied.

I acknowledge that I should avoid the use of Retin-A type products, aggressive exfoliation, waxing, and products containing acids that are no part of the recommended take-home regimen for 2-4 weeks following treatment.

I consent (to the best of my knowledge) that the answers I have given are correct and that I have not withheld any information that may be relevant to my treatment. I give consent for all future treatments

I fully consent that The Neon Salon may perform the treatment/procedure I have booked. I confirm that I am aware of any possible side effects this treatment may cause. I am fully aware of that I may not use specific products before this treatment, and I have been made aware of the pre and post treatment protocols.

I release The Neon Salon and its staff and suppliers of any liability associated with any injuries and /or current and future conditions resulting from any skincare procedures or products.  I am aware that any undisclosed information regarding my health cannot be held against The Neon Salon, it’s owners, employees or suppliers in the case of any mishap.

I confirm that I release The Neon Salon, its owners, staff and suppliers of any liability and that they will not be held responsible for any injuries, loss of life, loss and/or damage of my personal property or any unforeseen damage.

I agree that I have informed my skin care specialist of any health problems of which I am aware and all medications I am taking, particularly Accutane, Retinoids and topical prescription drug therapies.

I understand that skin rejuvenation is best achieved through a series of recommended treatments and compliance to the home care product program recommended by my Skin Care Specialist.

Informed Consent for General Facial, Skin or Body Treatments and Massages

I duly authorize The Neon Salon to perform any of the general facial, skin, and or body treatments or massages on me.

I confirm that I have informed the staff regarding any current or past medical condition, disease or medication taken and that the list of contraindications has been fully explained to me. I also undertake to inform the staff of any changes to my medical condition or medication taken during the course of my treatment or any further future treatments.

The Neon Salons general facial, skin and or body treatments may consist of surface cleansing, exfoliation (enzyme, glycolic, lactic, tca and salicylic peels), steam, extractions, corrective serum / serums, mask applications and day creams with or without spf, body scrubs, wraps and massage and aromatherapy. These treatments take approximately 30 – 90 minutes respectively to complete and are designed to restore skin health and beauty as much as possible in the scheduled time. Implements and equipment used in The Neon Salon treatment procedures are either disposable or properly sterilized according to the Health and Safety regulations. Our therapists may wear surgical gloves, face masks or eye goggles throughout the entire procedure to ensure sterile techniques.

I understand it is my choice to receive massage therapy and/or body treatments. I realize that the treatment is being given for the wellbeing of my body and mind, to relieve stress, muscular tension, spasm, or discomfort and increase circulation flow. I agree to communicate all physical concerns with my practitioner.

Informed Consent for Chemical Peel Treatment

I duly authorize The Neon Salon to perform the 15% or 30% Chemical Peel treatment on me. 

I understand that a Chemical Peel is a solution that harnesses the body’s natural processes of renewal, which may cause the skin to peel, allowing new skin to grow in its place and that this treatment is used for a variety of skin conditions of which I hereby consent to be a client receiving the Chemical Peel treatment on the specified area above for the specified concern.

I understand that clinical results may vary depending on individual factors, including medical history, skin type, client compliance with pre/post treatment instructions and individual response to treatments. 

I confirm that I have informed the staff regarding any current or past medical condition, disease or medication taken and that the list of contraindications has been fully explained to me. I also undertake to inform the staff of any changes to my medical condition or medication taken during the course of my treatment or any further future treatments.

I understand that there is a possibility of temporary Short-Term Effects which have been fully explained to me and that I will inform the staff of any adverse reactions within 24 hours of them occurring:

• Swelling or puffiness that may occur after 24-72 hours.
• Redness after 72 hours could indicate irritation or allergy to an ingredient.
• Itching can occur- oral antihistamine can be taken.
• Post inflammatory hyperpigmentation (PIH) can occur
• Underlying congestion can be brought to the surface- whiteheads, blackheads and pustules can occur.
• NB: For darker skin types, the skin may darken further before shedding.
• Note, pigmentation may worsen after the first few peels as it’s brought closer to the surface of the skin – therefore a course of peels is suggested.

I have been fully informed and understand:

• The treatment involves a series of sessions which need to be continued at set intervals dependent on the nature of the concern – failure to adhere will result in a less desired outcome
• The nature and purpose of the treatments
• Expected outcomes and possible short-term effects
• The final result obtained cannot be guaranteed
• The fee structure including the payment terms and conditions
• The late arrival, no-show and cancellation policies
• My condition is of cosmetic concern and that the decision to proceed is based solely on my expressed desire to do so.

I confirm and understand that the Pre-Treatment Protocols have been fully explained to me and are important and advisable for the desired outcome and that I undertake to / have adhered to these requirements prior to my treatment.

I confirm and understand that the Post-Treatment Protocols have been fully explained to me and I will adhere to these requirements after my treatment. I also acknowledge and understand that Maintenance and additional Home Care Routines are advisable for effective and long-lasting results. 

Informed Consent for Microneedling

I duly authorize The Neon Salon to perform the Microneedling treatment on me. 

I understand that Microneedling is an approved treatment, designed to create a safe healing response within the body, through the use of surgical microneedles that puncture pinpoint microchannels in the body, igniting natural collagen and elastin production from within the skin and is used for anti-aging, rejuvenation, pigmentation and skin texture of which I hereby consent to be a client receiving the Microneedling treatment on the specified area above for the specified concern.    

I understand that clinical results may vary depending on individual factors, including medical history, skin type, client compliance with pre/post treatment instructions and individual response to treatments. 

I confirm that I have informed the staff regarding any current or past medical condition, disease or medication taken and that the list of contraindications has been fully explained to me. I also undertake to inform the staff of any changes to my medical condition or medication taken during the course of my treatment or any further future treatments.

I understand that there is a possibility of temporary Short-Term Effects which have been fully explained to me and that I will inform the staff of any adverse reactions within 24 hours of them occurring:

• Swelling, rash-like effect and prolonged redness
• Sensitivity and stinging for more than 5 days
• Possible post inflammatory hyper-pigmentation (PIH) due to unprotected sun exposure 
• Mild bruising 
• Infection

I have been fully informed and understand:

• The treatment involves a series of sessions which need to be continued at set intervals dependent on the nature of the concern – failure to adhere will result in a less desired outcome
• The nature and purpose of the treatments
• Expected outcomes and possible short-term effects
• The final result obtained cannot be guaranteed
• The fee structure including the payment terms and conditions
• The late arrival, no-show and cancellation policies
• My condition is of cosmetic concern and that the decision to proceed is based solely on my expressed desire to do so.

Informed Consent for Dermaplaning

I understand I am receiving an exfoliation treatment using a sterile surgical blade.

I understand the possible side effects include but are not limited to skin tightness, mild to moderate redness, mild flaking, possible nicks.

I understand this procedure removes most, not all vellus hair (peach fuzz)

I understand the results of this treatment may vary due to conditions such as age, condition of skin, sun damage, climate, etc.

I understand this treatment is a cosmetic treatment and no medical claims are expressed or implied.

I understand that direct sun exposure, including tanning beds, is not recommended while undergoing treatment and the use of a daily sun block protection is mandatory.

I understand that any facial injections should be avoided 10 days before this treatment. I am not using Retin A and have been off Retin A for at least 3 days prior to treatment. I will call my practitioner if I have any questions or concerns about my treatment.

I have been advised not to exercise after my treatment.

If a chemical peel, Microneedling or any other treatment is included with this treatment, I understand that the sensation and penetration of the peel will be enhanced. This may cause skin irritation, mild discomfort, tenderness, lightening or darkening of the skin, infection, scarring, peeling, and activation of cold sores, when virus is already present in the body.

I hereby agree to all of the above and agree to have this treatment performed on me. I further agree to follow all post-care instructions. Prior to receiving any treatment, I have been candid in revealing any condition that may have bearing on this procedure. I am over 18 years of age

Informed Consent for Biominceur

I hereby understand that all treatments performed by The Neon Salon is of a cosmetic Slimming body wrap treatments on me. My desire to continue with the treatment, is my decision, and I am aware that clinical results may vary depending on individual factors, including medical history and client compliance with pre / post treatment instructions, and individual response to treatments. I have been made aware that my lifestyle will have a major effect on the treatment results, and that it would be beneficial for me to adapt my lifestyle, my diet & exercise regime, accordingly.

I take note that sunbed or spray tan sessions during any of my treatments are not recommended and should first be discussed with my therapist.

I certify that I have been fully informed of all treatment procedures, nature, purpose, expected outcomes and possible complications. I understand that no guarantee can be given as to the final result obtained. I acknowledge that it is my personal responsibility to inform the practitioner nature, and I duly authorise the practitioners of above-named clinic to perform Biominceur of any changes to my medical history before and during the course of any treatments, and I

confirm that should this occur; I shall advise the practitioner thereof.

I consent to the taking of photographs and authorise their anonymous use for the purpose of education & promotion. (Optional)

I am fully aware that my condition is of a cosmetic concern and that the decision to proceed is based solely on my expressed desire to do so.

I understand that the result of this treatment needs to be maintained by a healthy lifestyle, including a healthy diet and regular physical exercise.  

I certify that I have been given the opportunity to ask questions. Any questions have been answered to my satisfaction and that I have fully read and understood the contents of this consent form.

Informed Consent for iLipo

I authorize The Neon to perform the iLipo procedure for the purpose of spot fat reduction/improving the appearance of cellulite.

I am aware that clinical results may vary depending on individual factors, including medical history, client compliance with pre/post treatment instructions, and individual response to treatment. I have been made aware that my diet and the amount of exercise I do, will have a major effect on the results of my treatments.

If I do not make an effort to address my dietary requirements and exercise, I am aware that the results achieved may not be retained. I understand the treatment involves a course of treatments.

The fee structure has been fully explained and I understand that I am required to pay for a course of treatments prior to any procedures taking place. I am fully aware that should I wish to cancel the course, the outstanding treatment value is non- refundable.

I certify that I have been fully informed of the nature and purpose of the procedure, expected outcomes and possible complications, and I understand that no guarantee can be given as to the final result obtained.

I am fully aware that my condition is of a cosmetic concern and that the decision to proceed is based solely on my expressed desire to do so.

I consent to the taking of photographs and authorize their anonymous use for the purpose of education and promotion. I understand that at least 30 minutes of increased physical activity is recommended immediately following the treatment for maximum results. This is to help “burn off” released fat cell contents so it is not restored back into the empty fat cells.

I hereby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation. I acknowledge that my participation is purely voluntary and in no way mandated by The Neon.

I release The Neon Salon and its employees from any claims, demands, and causes of action as a result of my voluntary participation. I fully understand that I may injure myself as a result of my participation in this program and I release The Neon Salon from any liability now or in the future for conditions that I may obtain.

Informed Consent for photographs

I give permission for photographs to be used by the company and its technicians for educational purposes. Complete patient confidentiality will be maintained. The patient’s name or any other identifying information will be withheld. I consent to the publishing of photographs as long as my identity is concealed.

I understand the importance of Before and After Photographs and consent to the following: 

• The taking of my before and after photographs
• I authorize their anonymous use for the purposes of medical audit, education and promotion.

Consent for Current and Future Treatments:
